This is in response to some of the jobs in Local 477 requiring members to sign a daily self-evaluation form before starting work.
We have prepared the following notice and ask that you please add this very important section to the Self Evaluation form. “My answers to the questions above are to the best of my knowledge, but I am not a medical professional. My completion of this form does not relieve the General Contractor, (insert General Contractor Name) or my employer, (insert Electrical Contractor Name), of their responsibility to ensure I have a safe work environment.”
Please read through the form and contact a Representative at the Hall with questions.
We encourage all of you to keep the Union Hall updated on any changes at your job site so we may best address any problems or challenges faced by our Brothers & Sisters.
